A Retrospective Analysis of 160 Cases of Cholecystitis



Abstract
160 consecutive cases of histopathologically proven cholecystitis which had been treated surgically at the Department of Surgery of Pusan National University Hospital from January 1988 to December 1992 were analysed.
1) The peak incidence was 6th decade in both sex and the sex ratio of male to female was 1 : 1.2
2) The symptom duration less than 3 days was 29 cases(18.1%) and the onset more than 6 Months was 57 cases(35.6%)
3) The most common symptom and sign was right upper quadrant pain(97.5%), and tenderness(87.5%)
4) The leukocyte count above 10,000 was found in 100 cases(62.5%), alkaline phosphatase elevation above 92IU/l was 66 cases(41.3%), total bilirubin above 3.0mg/dl was 45 cases(28.1%)
5) Abnormalities of the X-ray finding of the simple abdomen was observed in 85 cases(53.1%) and among of those the gallstone shadow was identified in 12cases(7.5%)
6) Cholecystectomy was performed in 106 cases(66.2%), cholecystectomy with sphincteroplasty in 6 cases(3.7%) and cholecystostomy in 2 cases(1.3%)
7) Chronic cholecystitis was found in 71 cases(44.4%), acute cholecystitis was found in 89 cases(55.6%). The most comon complication of acute cholecystits was empyema which was 43 cases(48.3%).
Calculous cholecystits was found in 147 cases(91.8%), and acalculous cholecystits was found in 13 cases(8.2%).
8) The incidence of positive cultures from the bile was 72.7% studies at surgery. The most common bacteria was E.coli in 31.8%.
9) The postoperative complication rate was 22.5% and the most common complication was wound infection in 44.4%
